Notes & Instructions
Any footwear sent to ReChaco needs your Service Authorization (SA) number printed
clearly on paper placed INSIDE the box, as well as written clearly on the OUTSIDE of the
box. We also recommend that you keep this SA number for reference.
Please send only full pairs. Whether only one sandal or both needs repair, please send in both left and right sandal.
ReChaco accepts only CLEAN & DRY footwear.
Your sandals must be received within 10 days of completing this form.
Orders received beyond the 10 days will require us to contact you to update your repair
order. PLEASE NOTE, THIS CAN ADD 2 TO 3 WEEKS TO YOUR REPAIR.
Average in-house turnaround time for repairs is 10 business days after receiving sandals.
